Digital Public Infrastructure in the NHS
Draft Concept Note - August 2025
The NHS has 242 published, active, legally required digital standards for software vendors to adhere to, yet interoperability within its vast ecosystem remains elusive. What is missing? The current landscape of isolated systems and fragmented data, frustrates both healthcare professionals and patients, and stifles innovation.
The goal of this research project is to identify what is preventing these existing digital standards and protocols from delivering an interoperable digital ecosystem and to offer concrete policy proposals to improve the situation.
The primary output will be a published report, informed by a literature review to surface the existing understanding of what is and isnât working today, along with 10+ interviews with domain experts and two workshops to test the proposals and iterate based on feedback from active practitioners in the space.
The report will examine the political economy of the NHS and how it intersects with the tools and processes of software engineering to discover why a protocols- and standards-based approach to digital transformation has not taken off yet, despite evident investment in standards and devolved authority to choose different solutions, which should already be selecting for interoperability as a feature.

Emerging themes from the research that overlap with IIPP thinking
- Standards vs Autonomy - Trusts can ignore national level efforts if they dont deliver what they want. Local providers may selectively ignore legislated requirements to deliver a feature that is asked for. Once a system is deployed and used by healthcare workers it is hard to make the argument that it should be disabled because it doensât meet a legislative requirement. The trust wont hold the provider to account it if it does what they want. (source: MF)
- Cross region interop is always someone elses problem, never the feature that an individual trust wants to pay for (source: paraphrasing gordon guthrie and personal intuition)
- Funding - NHS digital projects still wrestling with capital based funding model that expects assets to be created. There are a massive 23,000 IT systems that connect to the NHS SpineâŠit is not uncommon for NHS trusts to have well over 100 official clinical systems across one organisation (Betton, 2022, p.17)
Electronic patient record providers stifle the market
Section titled âElectronic patient record providers stifle the marketâElectronic Patient Records (EPR) systems are a big piece of the puzzle. Large incubent companies move slowly, primarly responding to legislative requirements rather than innovating.
At the same time they stifle or purchase competition and related services, preferring to contintually expand the scope of their software (lock in) rather than offering ineroperable apis that encourage
- The software is mission critical. Itâs how a trust records and schedules interactions with patients.
- Lots of trusts use sotware from Epic or Cerner, both US-based companies.
- e.g Manchester University NHS Foundation Trust signed a 15-year contract worth ÂŁ181m with Epic in 2020 (Betton, 2022)
- In primary care, two IT suppliers, EMIS and TPP, dominate the market, providing electronic patient records to 95% of GPs. (Betton, 2022)

Fast Healthcare Interoperability Resources (đ„ FHIR)
Section titled âFast Healthcare Interoperability Resources (đ„ FHIR)âFHIR âŠuses a modern web-based suite of API technology, including a HTTP-based RESTful protocol, and a choice of JSON, XML or RDF for data representation. One of its goals is to facilitate interoperability between legacy health care systemsâŠ
FHIR provides an alternative to document-centric approaches by directly exposing discrete data elements as services. For example, basic elements of healthcare like patients, admissions, diagnostic reports and medications can each be retrieved and manipulated via their own resource URLs.
âStandards in healthcare data and systems are like toothbrushes,â he tells me, âeveryone has one, but nobody is prepared to share.â
However, clouds are making way for blue skies in the shape of a new standard for integration called HL7 FHIR. David Hancock (co-chair of INTEROpen) believes that HL7 FHIR, at last, gives a comprehensive, universal, open standard that really is a standard and not just a guide, as previous healthcare interoperability standards have been.â (Betton, 2022, p.288)

OpenHIE identifies HAPI FHI as a reference SHR (Shared Health Record) application, and FHIR servers are emerging as platforms that can function as shared health record services. These are implemented at scale in primarily high-income country HIEs, including in the United Kingdom (NHS), Canada, Australia, and Japan.

The Departmentâs previous attempt to reform how the NHS uses IT, running between 2002 and 2011, was both expensive and largely unsuccessful. We are therefore alarmed at how little progress has been made against current ambitions. The NHS missed its main target for a âpaperlessâ NHS by 2018, and this has now been watered downâŠ
Despite being recognised as essential to managing patient care, there has also been a lack of progress on interoperability. Only three out of the ten standards for interoperability so far identified by NHS Digital were ready by May 2020, and the national bodies are unable to tell us how many are now ready.
